Putting Members First

Guest Membership access within the state of California for members, spouses, or their dependents to keep their Vivity plan when temporarily residing outside of Vivity’s coverage area.3

2 Members can request a referral from the member’s primary care doctor for specialty care at a partnering Vivity Medical Group. Specialty care must be deemed medically necessary, and specialists must be contracted to see Vivity patients from a partnering Medical Group or IPA.
3 Must provide an address where the member is temporarily residing for 90 days or more. Requires an application which can be submitted with the assistance of a Vivity Concierge representative.
How Vivity Works
Vivity is an HMO health plan that brings world-class care to our members. Upon enrolling, members will select a primary care doctor (also called a PCP) who is responsible to manage and coordinate their Vivity patient’s care including preventative and primary care, as well as referrals to specialty care as needed. If a member needs to see a specialist, they can request a referral to a partnering medical group outside their home medical group where they see their primary care doctor.
To be sure our members are supported throughout their healthcare needs, Vivity offers dedicated support from our Vivity Concierge team and Nurse Patient Advocate. Vivity Concierge can assist members during open-enrollment periods to understand the network and benefits, and post-enrollment at the time of onboarding and ongoing throughout their plan coverage. Our Nurse Patient Advocate is a role exclusive to Vivity that assists members with referrals to specialty care and other access related matters.
